The meeting will be held in the 2nd Floor Party Room, beginning at 7:30pm. An election will be held to name three new Board Members.
We need to have a quorum to hold the meeting – 20% of the Unit Owners need to be represented either by Proxy or attending the meeting and casting a ballot. If the meeting is not represented by 20% of the Unit Owners, the meeting will have to be rescheduled, which can be costly.
If you can not attend the meeting, you will need to fill out a Proxy Form and either deposit it in the Proxy Box at the front desk or assign it to someone who can attend the meeting on your behalf.

Inside the packet mailed to Owners along with the notice, is a blank Proxy Form, instructions to participate in the meeting by Proxy, and candidate information forms. There were four Unit Owners who applied by the deadline to run for election to one of three 2 year terms on the Board of Directors. The election will be held the night of the 15th.
The four owners running for election are: Joe Guzzardo Unit 1801, Michael Parrie Unit 5401, George Pauley Unit 4811, and Jean Shamo Unit 4606. Meet the Candidate events are scheduled for Wednesday June 3rd at 7pm, Saturday June 6th, and Tuesday June 9th, all in the Party Room.
All owners are welcome and strongly encouraged to attend the meeting and cast a ballot for the election. The Association needs at least 20% of the ownership to attend, to have a quorum and be able to hold the meeting. If you cannot attend, please follow the instructions in your packet to assign a Proxy. Adjourning the meeting and sending out notice to hold another one can be very costly.

Dear Unit Owner,
The Park Tower Condo Association’s Annual Meeting of the Unit Owners will be held on Monday June 15th, 2015, at 7:30pm, in the 2nd Floor Party Room at 5415 North Sheridan Rd., Chicago, IL, 60640. Owners are welcome and strongly encouraged to attend and participate in the meeting.
At the Annual Meeting, the Association will hold an election to fill three positions on the Board of Directors for two year terms. The meeting requires a minimum 20% presence of unit owners for quorum, either in person or by executing a proxy. To participate via proxy, please follow the enclosed instructions and return the form with the addressed envelope. For those who own multiple units, a proxy for each unit will be enclosed.
Owners who attend the annual meeting can be counted towards quorum and vote with a ballot to be provided at the meeting. The Association’s auditor, hired to tabulate the vote and certify the results of the election, will be registering all ballots for use at the election. To receive ballots at the meeting, owners will need to bring a current government issued picture ID to confirm their identity.
Pursuant to the election rules passed on January 26th, 2015, this year’s election will be conducted by ‘Secret Ballot’. Ballots shall be marked only with the percentage of ownership for the unit, and the list of candidates and lines for write-ins. The auditor will register the ballots to owners and proxy holders who attend the meeting. They will also assign ballots to any proxy forms mailed without another individual assigned as proxy.
Owners who cannot attend the meeting and instead return the enclosed proxy form, can either assign another individual as proxy (where indicated), or leave it blank and the auditors will assign and cast a ballot as indicated on the form. If a vote is not exercised on the proxy form, the percentage of ownership will only be counted towards quorum.
Owners of record who submitted Candidate Application Forms by the May 8, 2015 deadline are listed as candidates on the proxy and ballots. There are blank spaces for write-in candidates. Nominations for additional candidates can be taken from the floor at the meeting. Candidates listed on the proxy have agreed to abide by the ‘Board Election Rules’ from the Park Tower Condominium Rules and Regulations Handbook. Included in the ‘Board Election Rules’ are the policies on campaigning, solicitation, and advertisement. Please contact the Management Office if you would like a copy. (over)

1. Wednesday June 3rd 7pm – Candidate Meet and Greet
2. Saturday June 6th 11am – Candidate Meet and Greet
3. Tuesday June 9th 7pm – Meet The Candidates Night
The candidate “Meet and Greet” events are intended as an informal gathering to meet, mingle and discuss issues in person with the candidates. The “Meet the Candidates Night” is intended as a more formal group meeting, chaired by an individual selected by the Board. Candidates will answer questions from members of the audience. Each of these events will be held in the 2nd Floor Party Room at 5415 North Sheridan Road, Chicago, IL, 60640.
Owners who cannot attend the Meet the Candidates Night, are invited to email questions in advance to parktowercondo@dkcondo.com. We will forward these questions to the meeting chair for consideration, and respond with the answers once they are made available. We encourage owners to attend these informative gatherings to meet the candidates and discuss issues with them.
Due to the nature of these events, it is likely a quorum of the current Board will be present and may be discussing business related to the operation of the Association. Voting on resolutions will not take place. However, consistent with the Association’s Bylaws and applicable law, since a quorum of Board Members is anticipated, please consider this formal notice of a Board Meeting at the above dates and times.
